Academic Enrollment

After completing the Inquiry Form, you will receive a username and link to join our online community through the Enrollment Management parent portal. Once in Enrollment Management, create an account and view one of the following checklists, depending on the grade information you provided on the Inquiry Form.

  • The following checklist items must be submitted to our Admissions Office:

    1. Complete the Enrollment Application

    2. A copy of original birth certificate (For the completion of the Birth Annotation Form)

    3. Proof of child’s Catholic baptism, if applicable

    4. Your child’s most current immunization record

  • The following checklist items must be submitted to our Admissions Office:

    1. Complete the Enrollment Application

    2. A copy of original birth certificate (For the completion of the Birth Annotation Form)

    3. A copy of your child’s IEP (Individualized Education Plan)/504 Plan, if applicable

    4. Proof of child’s Catholic baptism, if applicable

    5. Proof of Kindergarten completion (report card or certificate)

    6. Your child’s most current immunization record
